1.Incident/ Accident Report- Human Factors, Management Factors, Ship factors. (No blame culture). 2.Inform Flag, Class, P&l club (in case diversion to repatriate expected)
3.Dispensation required if Safe manning is questionable.
4.RCA including pictures, statement of facts etc.
5.CA/ PA
6.Sharing with the fleet/ Company vessels.
7.Extra ordinary safety meeting.
