As per ISM Code Section 5.1.5, the Master is responsible for reviewing the Safety Management System and reporting its deficiencies to shore-based management. In this report, Master records the effectiveness of the SMS onboard, deficiencies observed, non-conformities, near misses, accidents, audit findings, PSC observations, training needs, resource requirements, suggestions for improvement, and corrective or preventive actions required. This report is sent to the Company / DPA / shore management for management review, follow-up, and continual improvement of the Safety Management System.
